Please read the following statements, check, and sign and date the bottom.(Required)
- I understand the VA determines eligibility for education assistance in all cases.
- I understand that I must be formally declared in an academic degree program or an approved certificate program to utilize VA education benefits.
- I understand that any prior earned credit, including credit for military experience, must be evaluated by the College for transfer. If I receive transfer credit for a course that I have also completed at NCC, an adjustment will be made to my GI Bill certification. Adjustments to my certification may cause a debt to the VA.
- I understand that I am responsible for purchasing my books with the understanding that I will receive a book stipend to help offset the cost of my books, up to $1,000 per year. Any book charges applied to my student account, such as Follett Access fees, are not covered by the VA.
- I understand the VA will only pay for courses that apply to my current degree plan.
- I understand that if I change my hours enrolled (add/drop) or withdraw at any time during a semester, or if I make changes before the semester begins, I will notify the School Certifying Official by email immediately of these changes.
- I understand that if I withdraw from a course, including preregistering but never attending, my benefits may be subject to recoupment to the VA. I understand that I am responsible for any recoupment.
- I understand that if my educational benefits do not cover the total cost of my tuition and fees that I am responsible for the balance, and that if I owe a balance by the payment due date, my classes may be dropped
- I understand VA benefits may be discontinued if I fail to maintain satisfactory academic progress or attendance towards completion of my program.
- I understand that my NCC student email is the official means of electronic correspondence with the NCC School Certifying Official. I am responsible for checking my NCC student email regularly
- I understand that I must notify the School Certifying Official via email if I do not wish to be certified for a semester(s).
- I understand that my signature authorizes the School Certifying Official to release my enrollment transactions, academic standing, grades, graduation, etc. to either the Department of Veterans Affairs or Department of Veterans Services for the purposes of processing payments or audit of record.
