Testing accommodations are available on the Accuplacer test for those with a documented disability. To Request Accuplacer Testing Accommodations, submit this form:

• IEP
• 504 Plan
• 3-Year Evaluation (educational testing)
• Neuropsychological Evaluation Report
• Letter from physician, psychologist, or psychiatrist stating diagnosis and needed accommodations.
